Cosan Raises $15M to Expand AI-Powered, Human-Centered Virtual Care for Chronic Conditions
Cosan, a connected chronic care management company that helps healthcare providers extend care beyond the clinic, has announced a $15 million strategic growth equity investment led by Ziegler Link-age Funds and Specialty Alliance, the nation’s largest provider of specialty care and a long-standing partner of Cosan. The funding will accelerate the expansion of Cosan’s AI-enabled, team-based virtual care platform, designed to deliver proactive, personalized, and compassionate support to patients managing chronic conditions between in-person visits. The investment will enable Cosan to scale its multidisciplinary care teams, enhance its AI-driven insights, and support healthcare practices aiming to improve patient outcomes while increasing operational efficiency. By combining advanced automation with human-led care, Cosan ensures that patients receive timely, individualized attention without compromising clinical quality or empathy. “We’re proud to deepen our partnership with Cosan,” said Katie Schmitz, Managing Partner at Ziegler Link-age Funds. “Cosan is demonstrating how AI can amplify the human touch—closing care gaps, strengthening patient-provider relationships, and delivering outcomes that truly matter.” Dr. Paul Berggreen, Chief Strategy Officer at Specialty Alliance, added, “Cosan has been an exceptional partner in supporting our patients between visits. As we expand our services across gastroenterology, urology, rheumatology, and oncology, their chronic care management model remains an essential resource.” Cosan currently serves more than 130,000 patients nationwide and enrolls approximately 300 new patients each day. Its platform uses AI to analyze patient data, identify emerging health risks, streamline clinical workflows, and personalize patient engagement—empowering care teams to focus on meaningful interactions while maintaining continuity of care. “Our mission is to help providers deliver compassionate, high-quality care beyond the office,” said David Hunt, CEO of Cosan. “AI allows us to extend the reach of care teams, not replace them. This investment strengthens our ability to deliver connected, timely, and personal care that improves lives.” Cosan’s model aligns with the healthcare industry’s growing shift toward proactive, value-based chronic disease management—where technology and human connection work together to deliver better health outcomes.
